Characteristics:
As many as possible of the plasma proteins are removed from the erythrocytes, leukocytes removed through washing with NaCl 0,9% or SAGM, after which the erythrocytes are resuspended in approximately 100 ml SAGM. The concentration of erythrocytes is a minimum of 135 ml (40 g Hb) and the haematocrit 0.50 – 0.65 L/L. Due to the washing, the units contain, inter alia, very little IgA, allergens and complement. If the washing is applied for a patient with IgA deficiency, the plasma protein in the final washing fluid must be < 0.2 g per product.
Washed erythrocytes can be stored for 5 days when resuspended in SAGM with the aid of a closed system.
Indication
Erythrocyte transfusion in patients with:
- IgA deficiency and IgA antibodies that have furthermore shown a serious reaction to the administration of IgA-containing blood products (these must be washed 3 to 6 times for this);
- repeatedly occurring serious allergic reaction against plasma proteins/allergens (a single washing is usually sufficient for this).
